The Lead Unit Clerk will be responsible for preparing and compiling records in the skilled care unit, and for performing clerical functions including verbal communication with visitors, Residents and other Team Members in person, by fax, e-mail or phone. Assist Admission/Discharge Nurse, Charge Nurse, Social Worker and/or Clinical Manager with paperwork, including but not limited to admissions, discharges, transfers and physician orders.

- $20/hour + based on experience
Responsibilities:
- Assembles and prepares medical record charts for newly admitted Residents.
- Prepares appropriate door and name tags for new admissions.
- Makes identification bracelets for new Residents and replaces identification bracelets as needed on current Residents.
- Takes photographs of newly admitted Residents, and updates current Resident photos PRN for identification in medication administration system, charts and elopement file.
- Files hospital records of Resident in proper section in Residents’ chart, and files other pertinent documentation appropriately in chart.
- Emails admission/discharge/room changes to appropriate community Team Members and Team Leaders.
- Compiles census of Residents.
- Sets up Residents’ physician appointments, notifies families, arranges transportation and maintains Outlook Calendar allowing IDT access to appointment information.
- Prepares envelopes with appropriate documentation for Residents going out for physician consult.
- Requisitions supplies designated by nursing team.
- Completes daily and weekly reports for nursing.
- Maintains adequate supply of documentation forms.
- Answers telephone and provides information or relays messages to Residents and interdisciplinary care team.
- Directs visitors to Residents’ rooms.
- Distributes mail, newspapers, and flowers to Residents.
- Takes menus to Resident rooms.
- Completes and logs lab and x-ray request slips under the direction of a licensed nurse. Orders labs through computerized system. Calls lab and/or x-ray as requested by nurse.
- Responds to call bells as quickly as possible, summons assistance or assists as appropriate, without providing hands on care unless qualified to do so.
- Assists nurses with supervision of high risk fall Residents.
- Attempts to redirect confused or wandering Residents from leaving the floor.
- Resets alarms activated from wandering Residents only after ensuring that all Residents are accounted for.
- Mails and files "Physician Order" slips, faxes verbal order for signatures. Tracks verbal orders to ensure compliance.
- Ensures receipt of consulting physician dictation.
- Copies physician orders to RNAC and pharmacy.
- Takes off physician orders, test and lab results for the approval of the Licensed Nurse. Notifies Nurse of critical lab results.
- Prepares for physician visits (pull charts, consult sheets, list of Residents to be seen, etc.)
- Creates Fond Memories booklets for deceased Residents. Posts memorial announcement and removes within 10 days.
- Copies and forwards oxygen records to the Business Office for proper billing.
- Prints and files previous month’s Physician Order Sheets in Resident charts.
- Coordinates in house Beauty Shop appointments.
- Requests and contacts maintenance PRN for repairs and concerns.
- Confers with the Administrative Assistant and helps to facilitate any special instructions or assignments.
- Acts as a reference to, and fields inquiries from, other Unit Clerks.
- Responsible for training of new Team Members.
- Provides input for performance evaluations.
- Responsible to ensure Unit Clerk schedules are complete and arrange coverage for absent Team Members, as necessary.
Supervisory Responsibilities:
- Assumes oversight responsibility with the Administrative Assistant.
Qualifications:
- High school diploma or general education degree (GED)
- One year experience with Microsoft Office
- Familiarity with medical terminology

- Week #1 8:00am-4:30pm on Sunday, Monday, Tuesday, Thursday, Friday
- Week #2 8:00am-4:30pm on Monday, Tuesday, Thursday, Friday, Saturday
- Occasional coverage for the Concierge Desks on Holidays
